Health Partners Initial Appointments
It is important to contact Health Partners as soon as possible regarding any new physician(s) joining your group. In those instances where malpractice coverage and start dates are not yet in effect, we will process all potential new physicians as pending receipt of all required information.
If a physician is applying for Mount Carmel Health (MCH) affiliation(s), please make sure to indicate to the Mount Carmel Health Credentialing Verification Office (CVO) that the physician is also seeking Health Partners participation. Upon completion of all verifications performed by the CVO, the CVO will forward required documents to Health Partners for Committee approval to our network. All physicians applying for MCH affiliation will have their Health Partners’ participation approval pended until appointment is verified.

Health Partners Reappointment
As a requirement for participation in the managed care contracts that are offered as a benefit of participation, recredentialing is required to be performed every two years. Three (3) months before the due date of your recredentialing cycle, you will receive a request from the Mount Carmel Health System CVO for recredentialing documents on our behalf. If you plan to continue participation in Health Partners, all documents must be returned to the CVO for processing. Upon completion, they will forward your completed documents to Health Partners for review. Failure to respond to the CVO’s reappointment request could change the status of your participation in the managed care contracts you have accepted through Health Partners.
