| How to Join Health Partners |

The first step in joining Health Partners is to complete an application packet and return the application to Health Partners. Physicians should submit payment for their Health Partners share at this time. If you are not approved for Health Partners membership, your shareholder payment will be returned. Please refer to the .pdf document titled Subscription Agreement below for futher information. The application is reviewed and the physician applicant will be notified if there is any additional information that needs to be submitted. When the application is complete, physician applicants are presented to the monthly Health Partners Credentialing Committee for approval. Once approved, your Provider Relations manager will provide you with an accept/reject form for each managed care contract that Health Partners holds. The accept/reject forms are sent to Provider Relations and the necessary demographics are provided to each managed care plan accepted on behalf of the physician.
